3. Consent to audio transcription / AI recording:
Your clinician may use secure software to audio-record and transcribe your consultation to assist with accurate documentation.
• The software listens to the conversation and generates a text transcription only.
• No audio is stored.
• The transcription is securely stored for a short period and is accessible only by authorised clinic staff. • Your clinician verifies all notes before they become part of your permanent medical record.
You may decline or withdraw this consent at any time, and this will not affect your care.
4. Your rights - You may:
• Request access to your medical information
• Ask for corrections if information is inaccurate
• Withdraw consent to use or disclose information (unless already lawfully used) • Ask how your information is stored or handled
All records are kept securely in accordance with the Privacy Act 1988 (Cth) and relevant WA privacy principles.
